Together, we’ll create a personalized plan that helps you move forward, communicate clearly with providers, and feel more prepared for what’s ahead.

  • Three 60-minute sessions 
     

  • Customized written transition roadmap (medical, social, or educational)
     

  • System navigation support (e.g., understanding “school speak”, DD services, referrals, timelines)
     

  • Collaboration planning with schools, service providers, or other supports
     

  • IEP or 504 Meeting attendance (virtual preferred)
